  • Self Managed

    Oz Medical PPE is an NDIS register provider. If you are self managed, fill out our contact form and advise of the products you'd like to purchase and we can provide an invoice to claim through the NDIS.

  • Plan Managed

    If your NDIS Plan funds a Plan Manager, billing can be organized through them upon receiving a completed Biller Authorisation Form from the Plan Management Agency. Please provide your contact information in the forms below.
